CPE at Redcare Pharmacy’s First CDL Session: Designing a Digital, Tailormade, At-Home Future for Pharmacy

On 2 July 2025, CPE took part in Redcare Pharmacy’s inaugural Session in Brussels. The event focused on reimagining the future of pharmacy through the lenses of digital innovation, personalization, and home-based care — with patients at the centre.

CPE Senior Policy Officer Joséphine Mosset delivered a thought-provoking intervention on how pharmacy services must evolve to meet the complex needs of people living with cancer. Her contribution spotlighted key pillars of a future-ready, patient-driven pharmacy model:

    • Bridging hospital and home: Advocating for seamless data exchange between hospitals and community pharmacists to support safe transitions to home-based self-care.
    • Personalisation as safety: Calling for real-time, dynamic patient summaries — enriched by patient-reported data — to enable pharmacists to flag drug interactions and cumulative toxicities early.
    • Trust in digital pharmacy: Urging Member States to uphold their obligations under the Falsified Medicines Directive (FMD) by running awareness campaigns on verifying legitimate online pharmacies.
    • A discharge medication profile that travels with the patient: Emphasising the need to hardwire pharmacotherapy continuity into the European Health Data Space (EHDS) by making the ‘discharge medication profile’ a mandatory dataset for all pharmacies.

Josephine emphasized that when pharmacists are equipped with a complete, patient-centred dataset through the EHDS—including patient-reported information and over-the-counter choices—they are better positioned to provide personalised and safer care. With this holistic view, tailored pharmacy services can move from being the exception to becoming the standard across Europe.

CPE remains firmly committed to ensuring that the digital evolution of pharmacy strengthens — rather than fragments — the patient journey. We continue to advocate for systems that are effective, inclusive, and ethically grounded, empowering patients to co-shape the tools and services they rely on every day.
